I am experiencing the aforementioned error when I construct a custom form in DHIS2 version 2.40.4.1. Furthermore, I made the identical form in the same tracker instance, and it functioned perfectly. Could someone who is experiencing the same error help me?

Thank you for warm welcome to DHIS2 Community. In one case, I developed data elements and an event program. I then made a custom form using that event program, however when I try to test it in the capture app, it doesn’t work!
I would like to build the form that is attached and note I must have eighteen buttons for standards, and when I click on any of them, I can see the same table with different information about that standard. I would appreciate your support. Thank you

Would you please take a screenshot of the program configuration? And what’s not working? Please make sure you’ve checked the access and sharing settings for the program and the user’s OUs.
