Tracker program stage layout changed – how to restore classic form view?

Hello everyone,

I’ve noticed a change in the display of one stage in my Tracker program.
Initially, the “Confirmation of care” section looked like in the first screenshot (a clean form layout with separated input fields and clear alignment).

However, it now appears as shown in the second screenshot — in a horizontal table format with yellow cells and an error message saying “Value type not supported: PHONE_NUMBER.”

I haven’t changed any program configuration or metadata, so I’m not sure why the layout has changed.

Question:
How can I restore the original display (the classic form layout) as shown in the first screenshot?
Could this be related to an update in the Capture app or to a modification in the field value type configuration?

Hi @elmoujarrade

Looks like you’re using the deprecated Tracker Capture app … (just a reminder, it might be important to plan and transition to using the Capture app, please see Implementation - Implémentation > App Transition and Version Upgrades )

Either way, I think the reason the form view changed is that somehow the ‘grid form view’ was clicked but you can revert back if you select ‘default form’. See screenshot below:
