To set up users… is it only possible via addition of a user from the User App by someone with the proper role? Or is there a way for a person to go to the url of the login page of an instance and request access to the system (via a “Register” app or something like that?)

It can be done either way. It is just that the user role for the Register might just have very limited access as no one wants to leave their open to the world.
So if configured, you can allow Self-Register.

as Knut says a good option here is to use the "self registration" feature.
First you set up a user role with very limited access. You can enable
self-registration from apps > settings > access > Self registration account
user role > set it to the appropriate role. Now anyone can self-register
for such an account. You can then have them send a DHIS message / email /
call to a DHIS administrator and ask for increased permissions in the
system, if necessary.
On a related note, you can also do "user accounts invitations" from the
user module (or through the web api) where you can send account invitations
over email to people.
