Which version are you using?
I also had the same problem when I updated from version 2.39.1 to 2.39.5.
Fortunately I upgraded to version 2.40.3.2 and no longer had the problem.

I use version 2.40.4.1.

Check the names of all options in the option sets you are using in the form if there is space before or after the name as shown in the screenshot. If so, remove the spaces and try again.
