Optimizing DHIS2 for Diabetes Mellitus Screening & Non-Communicable Disease Management

Introduction: Diabetes and Hypertension are significant public health issues in Sri Lanka, requiring effective screening, early detection and timely intervention. Diabetes Compass (DC) community screening and clinic apps were developed to support early detection and management. DHIS2, recognised as a digital public good (DPG), utilizes visualization features to deliver real-time data insights, and analytics, and enable evidence-based decision-making. Objective: This study explores the process of utilizing DHIS2 within a national digital health ecosystem, integrating heterogeneous digital solutions. The focus is on examining how DHIS2 facilitates seamless data exchange, enhances system interoperability, supports informed decision-making across diverse health applications, supports early detection, and improves the quality of care. Case Study: The DC community screening and clinic apps, developed using OpenSRP with support from key multi-stakeholders, integrate seamlessly with the National NCD Data Warehouse on DHIS2 through FHIR-based integration by HISP Sri Lanka. DHIS2’s customizability enables complex cohort analytics and flexible visualizations. In a heterogeneous digital environment, data is captured through external solutions into DHIS2. Its adaptability ensures multi-stakeholder support, facilitating data-driven decision-making and improved health outcomes. Results: Initially implemented in seven Grama Niladhari divisions in Kalutara District, the DHIS2 dashboard tracks critical indicators such as screening coverage, diabetes risk, and treatment results. These insights drive data-informed decision-making, promote early detection, reduce complications, and help lower healthcare costs linked to late-stage diabetes. Lessons Learned: FHIR-based DHIS2 integration with NCD data capture solutions via OpenHIM enabled real-time data transfer and visualization, improving screening, risk identification, and patient outcomes. Advanced analytics enhance long-term NCD management through decision-making, lower cost and early detection.
