I was asking about the build revision number of your 2.15. Please always include ths when you have an issue.
You will find the build revision under Profile, then select About DHIS2 It will be a 5 digit number, e.g. 15153

Can you send me a screenshot or copy paste of your console. (In case you don’t know, on chrome that would be — right click → inspect element → console tab — and it would look something like the screenshot.)
Additionally could you perhaps send me the output of your https://apps.dhis2.org/demo/dhis-web-commons/menu/getModules.action content.
Also what could help is if you send me the url you access your dhis instance on.

Actually, if you have Android phone or Java enabled phone, you should consider using the app. Both the android app and j2me app does not require you to re design the form for mobile. All of the configuration can be done from server side.
For the android app, you can download it from PlayStore: https://play.google.com/store/apps/details?id=org.dhis2.mobile
The J2ME app please download from DHIS download page: https://www.dhis2.org/downloads
