It is very difficult to off-hand say what would cause this. It would help if you share the DHIS2 version you are using and other error messages that might come up in the log file or the console.

As @Elmarie_Claasen suggested please share the version of the DHIS2 instance you are using. Additionally, would you kindly share the entire Catalina.out log (without sensitive/authentication info)?

Thank you for looking into this with me. I realized that the two screenshots were not uploaded properly earlier. Please find the missing screenshot below.
I am using DHIS2 version 2.41.5. After further investigation, the issue was traced to a specific organisation unit on the instance. From the error logs, it appeared that the system was attempting to process the same organisation unit ID twice, which should not be possible.
We also checked the database using pgAdmin to confirm whether there were duplicate records, but none were found. This indicated that the issue was likely a bug.
The good news is that the issue has now been resolved. Restarting the server did not fix it. However, I moved all organisation units under the affected organisation unit to a newly created one, then deleted the affected organisation unit. This resolved the issue.
