How to see LGA, Ward, and other organizational units in data output

I am pulling some data at the health facility-level but because there are several health facilities with identical names between LGAs within the same state, I am hoping to see my output with columns listing the corresponding LGA and Ward for each facility. I am new to this system so this could be a super easy question but I have not figured it out yet. When I drill down to the health facility level it no longer provides the LGA or Ward name. Any advice on how to see an output like below would be so helpful.
