District Reporting Rate not Correct

I have a problem with reporting rate for levels upon HFs; I meant districts, governorates, and country. For example, if I have district A and its HFs from 1 to 11, all of them provide family planning services, except HF #11, when I issuing the reporting rate from the data visualizer and reports app; It appears that all HFs (named from 1-10) are 100%, but the district A reproting rate is 94%. After I reviewed it carefully, I discovered it also accounts for HF #11, despite it doesn’t provide family planning services. Also, the show the expected reports is 11
How can you help me to fix this issue?

The denominator for reporting rates is normally based on the Dataset forms you assign to each facility, but I assume you’ve already checked that, and the family planning form isn’t assigned to HF #11?

Perhaps the other thing to check is that the form hasn’t accidentally been assigned to the District itself - this would then also count as a non-reporting entity when calculating district totals.

After I checked, I saw the district name assigned with service wrongly, as this service assumes to be assigned to HF level only. However, it works well now
