DHIS2 Critical Incident Reporting for Safer Care
Humanitarian hospitals require practical ways to learn from critical incidents without increasing administrative burden or the fear of blame. We developed and implemented a Critical Incident Reporting (CIR) solution on DHIS2 in ICRC-supported hospitals to enable secure, rapid, and standardized reporting of incidents and near-misses. The tool captures structured details to analyze underlying system factors—such as process, communication, staffing, equipment, and environment—allowing the Hospital Clinical Governance Group to translate data into concrete corrective actions, including policy updates and targeted training. To ensure feasibility for frontline staff, we built a dedicated “Forms” application. This tool streamlines reporting into a minimal workflow: users can reach the correct form quickly, submit a structured report efficiently, and generate a copy for safekeeping and future reference. To accelerate review and follow-up, we integrated an Event Report Widget that lists incidents by action status (identifying who takes the next step). This widget supports rapid filtering and provides direct links to records, enabling management to provide feedback within seconds. In parallel, we configured DHIS2 program notifications to automate the feedback loop: on submission, the Clinical Governance Group receives an alert with a direct link for review; once feedback or actions are recorded, field management is notified, improving accountability and shortening turnaround from days to minutes. Furthermore, CIR supports capacity building by converting real-world reports into practical learning content for onboarding and refresher sessions. This creates a continuous feedback loop where lessons learned are reinforced through updated guidelines. Collectively, these components promote a non-blaming reporting culture, improve early risk detection, and enhance safety for patients, staff, and visitors.
