Please check that the sortOrder in all the related metadata is unique and correctly sorted as this might be the issue. You might need to modify this in the database directly. @ctejo faced a similar issue: Cannot open Capture App - #5 by ctejo
Also, after you do all the debugging and fixing, please test using the browser’s Guest mode [bulletproof way to avoid browser cache issue] and I assume you’re also using the Maintenance clear application cache in the Data Administration app..
