Creating Report Summary

Hello everyone
Can anyone help me with how to generate a summary report based on the answers from the form I created?

I want at the bottom of the form I created to have a Summary of some of the questions and their respective answers. thanks

Sorry, it’s not clear which form you are talking about. Data sets? Tracker or event program form?

In Tracker and Event programs it’s possible to create program indicators and choose to display them on the form. The same thing is possible in the data sets using an indicator and adding it to the dataset form design.
