Configuring Cause of Death Module

Hello,
I have successfully installed the ICD-11 Cause of Death Module (Developed by HISP-Vietnam) but encountering an error on the second part of the form.
image. I am able to enroll a death successfully.

The potion below (Frame A and B) cannot generate the code, and on saving, the page becomes blank and data is lost

Anyone who has encountered this or could point me in the right direction.
