Can not save custom form

Dear All,

I am not able to save the custom data entry form in DHIS2 version 2.11. After designing the custom data entry form and clicking the save button when I want to see the custom data entry form the design is not showing.

Also I can not insert the data element into data entry form.

I have cleared cache also.But still the problem is there.

Please suggest me what should I do.

Are you sure the dataset is assigned to your user role?Does it appear in the dropdown?
