In data entry design form how to add two or more fields

eg. pregnancies

  1. no. of pregnancies in PHC
  2. no. of pregnancies in CHC
  3. no. of pregnancies in TALUK
  4. no. of pregnancies in district

Total pregnancies = 1+2+3+4
