Android App-Data capture app didn't show all forms

I have create data capture on Data entry form look like below

How ever when I use data capture app on android device. It didn’t show all data capture only A-activity; it didn’t show A-meeting-cate com form showed below:

Any help ? I am appreciate for help and thanks
