- Delete soft deleted records
- Clear cache
But still the problem persists.

Thank you for your post. What version of DHIS2 is the instance you are using? What is the version number of the app?
Additionally, was the data imported?

Could u please explain in details how the duplication happened. I meant did you execute importing data or did you insert the data one time and the second record duplication automatically, or … etc. Also what is version of DHIS2?
You may not have permission to delete records
the same happened to me last year
It was sold only by upgrading the current version of the DHIS2.
Otherwise I couldn’t find any reason for this behaviour.
