What is the best way to follow up mothers visiting independent Mother-Child health care centers
We would like to follow-up mothers during pregnancy, childbirth and post-natal care. Each stage takes place in a different entity. A mother (and, for post-natal care the child) is seen several times in the pre-natal and post-natal clinic and once in the maternity ward.
A mother may be seen initially and registered at any stage. Stage specific forms need to be filled in at every visit.
While we hope that the ID of the mother will be available at each of her subsequent visits, the health care worker would probably often need to perform a search on the name when it is not available.
We wonder how best to set this up in Tracker. The history of the mother should be available at every visit, so we want to see details of pre-natal visits and birth at port-natal care visits.
Should there be one single programme in which we chose the appropriate forms for each stage ? This would enable us to search for mothers within a programme and see her history.
Or would it be better to have three different programmes and somehow set up search between forms and see information on previous visits in other programmes ?
Additionally, have you considered following the official programs’ designs? For instance, for your case, I recommend comparing and analyzing the Antenatal Care (ANC) Tracker program.
If each stage happens in a different OU. You will be using the transfer TEIs feature.
In this case, the program stage can be set to a repeatable stage. And it’s possible to create a form for each program stage (if you mean it needs to be unique in each repetition then depending on the uniqueness it could be handled using program rules).
When configuring the TEA, it’s possible to select which ones are searchable so the health care worker will be able to use both ID or name as well as any other TEA that are set to searchable.

It’d be great if you share an update on how you’ll be implementing this including the TEIs, access levels, OUs, and relationships. This will depend on the information you will be collecting.
