Select option in aggregate data entry

Greetings

I am trying to design a system that capture veterinary information, and this includes text.

I have designed the data elements and their respective option sets, but when I go for data entry I cannot select the option in the drop down list.

May you please assist

Yes I did assign. The list is showing but you cannot select anything
