Dear Colleagues.
I have the following problem after migrating from version 2.11 to 2.12 in individual records / events anonymous:
When trying to edit the form, the names of dataelements not appear next to the checkbox.
When trying to insert information into the form, the system does not open for typing.
Below are the screens of the problem 1 and problem 2.
Can you explain what happened?

Hi José,
Unfortunately, the log does not contain anything of interest. But I notice you are running DHIS2 on Windows, in which case the error messages would likely show up in the DOS command window which opens as you start Tomcat. Do you see any error messages there, and if so, could you paste them here?

Do you see any error messages in your Tomcat log? If so, could you share the relevant part of it?
Also make sure you have cleared the cache of your browser and tried Chrome Incognito mode.

Hi Jose,
I had the same problem as your image of problem 2. When adding a new entry, the fields did not appear. The happened any time I changed the individual records data set by adding or subtracting data elements. I was able to resolve it by restarting the DHIS-2 instance. In the log we saw a null pointer error suggesting that the system was unable to find one of the new fields. We tried deleting cache (local, server and nginx), but only a restart would solve the problem.

Hi Jose,

Hi Knut.
The application is started by the monitor Tomcat 7 and no warning message appears on screen DOS. I have the log are saved by Tomcat beyond “Catalina.txt” I sent for you. Here are the other logs. Hope that helps.

Dear All,
I face same problem. I am using 2.12 version, PostgreSQL and UBUNTU. When i edit any form from application or any other change from application. After changes the data entry form do not come.
I solved this problem manually, After editing the form, Directly i update the table programstage column capturecoordinates type boolean as FALSE.
UPDATE programstage
SET capturecoordinates=FALSE
WHERE programstageid = 3;
Then restart the tomcat, then the form coming properly. I saw when i update the event in 2.12 version this column capturecoordinates value is empty.
Hope the problem will solved soon.
