I am using DHIS2 Tracker for managing the health data population in villages around a hospital. The organisation units are arranged in a hierarchy of 3 levels as given below.

After training the hospital staff on configuring organisation units and let them practice on the system I ended up with a weird problem.
Although the hierarchy levels are intact, the org units are rendered in a wrong structure in the search filter of tracker capture. Please find the screenshot.
You can see that things are properly arranged in the left pane, but not the same in the search criteria. It shows the corrupted structure also in Maintenance-> Org units → Hierarchy operations. But after checking the database I do not see any problem with the org unit hierarchy level and the parent org unit.
What could have gone wrong?
I can definitely recover the system using an older backup. But I am wondering how to fix this if it happens again in the future.
Hi! It might be something related to how you configured the entry and output orgunits in your user set up. Could please provide a snapshot of your user edit screen?
