Issue in tracker capture while adding relationship

Hello Team,
Greetings of the Day,

We are facing the issue in tracker capture, version 2.28 while adding the relationship.

Whenever I’m trying to add relationship for any record first it will get added but when i update any tracked entity attribute value and refresh the page than relationship will automatically get deleted from the record. Kindly take a look of this. I hope for the positive response from you.

I tried the similar thing on demo version (https://play.dhis2.org/2.28/dhis-web-commons/security/login.action) and faced the same problem.
