From paper form to dataset hints

Hi all,

I am wondering if you could help me build the data set for the following paper form.

On the paper form, we register the total number of participants, number of female and male participants; and position each participant holds.

Here is a very simple dataset that you can use. I have not added totals as they can be calculated.
Participant male
Participant female
Position Director
Position Chief Medical Officer
Position HoD
Position MCH
Position Pharmacist
Position SESP
Position Other
