I dont think this is possible in dhis2, but let me ask.
I would like to create a custom form with ‘dynamic fields’ just like a spreadsheet file with 4 columns (A, B, C, D) then i can enter data in rows (A1, B1, C1, D1), then i click ‘add button’ (or a new row showup automatically), then i get another row to enter some data (A2, B2, C2, D2) again and again and again until nth row.

can you please be a bit more specific about the requirements here,
maybe provide a mock up of the data entry form?
Ola and I were actually discussing this kind of data entry
functionality last week where you can select data elements
dynamically. This might be useful for instance when you have a large
set of diseases as data elements in your system but typically capture
data for only a few of them.
