Can you help and see if DHIS2 can create these questions and answer into Data Entry?

I would look into Events or Tracker rather than Data Entry (Aggregate). I see that you have skip logic requirement (question 110).

Create your questions as tracker data elements and your answers as options set. For example, question 108 Where do you study and the answers college or highschool, tertiary, etc…

  • create an option set ‘Education level / School type’
  • add the option answers (college, university, etc.)
  • create a data element ‘Where do you study’
  • set domain type as ‘Tracker’
  • Create a program (events or tracker) - depends on your need
